Program Description

Thea Bowman Apartments is a permanent supportive housing program that serves single adults and couples without children transitioning out of homelessness. Thea Bowman has 36 units set aside for Veterans and 44 units for single adults. As a Housing First, Harm Reduction program, Thea Bowman offers compassionate, dignified support that emphasizes safety and stability and encourages sustained growth in independent living skills. We do so in an environment of promise that welcomes diversity and honors each person's unique experience.

Responsibilities

This position is responsible for proactively engaging a caseload of at least 30 residents. Duties include intake assessments, referrals, and supporting residents with maintaining their housing through the lens of Harm Reduction and Trauma-Informed Care.

Case Management
  • Coordinate new resident intake and case management plans. Assess resident needs and required level of services, identify community resources to meet those needs, facilitate residents' access to services and advocate for residents on a continuing basis.
  • Provide case management services to residents including, but not limited to:
    • Outreach during times of crisis and as part of Thea Bowman's Housing Retention team
    • Accessing financial and health benefits
    • Primary health care
    • Mental health treatment and/or chemical dependency counseling
    • Home Health Care / COPES
    • Protective payeeship
    • Transportation
    • Other services (food benefits, legal services, etc.)
  • Develop housing services plans with the residents, with a focus on harm reduction, housing stabilization and goal attainment. Meet regularly with residents to discuss these goals, and update the plan annually.
  • Foster a sense of community and support among Thea Bowman residents. This includes organizing and/or facilitating events, activities and outings.

Community Resource Services Coordination

  • Support residents in community-building efforts and activities.
  • Organize and plan educational, advocacy and health workshops for residents.
  • Coordinate access to resources with the Resident Services Staff to assist tenants with getting to appointments on time and getting resources based on Front Desk staff's observations/interactions as recorded in Shiftnotes.
  • Develop and maintain relationships with other agencies to facilitate referrals to appropriate community resources.
  • Attend monthly CCS/CHS King County Case Manager meetings.
  • Represent Thea Bowman in service provider networks, including committees, coalitions, advocacy groups and/or employment, volunteer or housing fairs, etc.

Administrative & Program Support Duties
  • Maintain and review case records and documentation in files and log.
  • Maintain case management statistics in compliance with HMIS.
  • Maintain resources in files available to other staff and residents.
  • Complete monthly/quarterly/yearly demographics reports for government and private funders.
  • Participate in required staff meetings as scheduled, including Client Meetings, Admin Meetings, Case Management meetings, and All Staff meetings.
  • Provide information to staff around relevant case management issues and best practices.
  • Represent Thea Bowman in service provider networks.
  • Assist in the supervision of program volunteers and student interns.
  • Develop and safe-keep a workplace that values and supports a culturally diverse work environment.

General Responsibilities
  • Maintain accurate record of hours worked and turn in timesheets on schedule.
  • Attend trainings required of employees.
  • Maintain confidentiality of program, staff and clients.
  • Maintain cooperative relationships with residents/businesses in surrounding neighborhood.
  • Contribute to the mission of Catholic Community Services and Catholic Housing Services.

Job Conditions

This position requires the employee to work in an environment where there may be exposure to blood, bodily fluids and other potentially infectious material. Other exposures could include cleaning supplies, chemicals involved in pest control, paint and other materials used in building maintenance. Working conditions may include interruptions, working alone, interactions with angry persons and exposure to computer CRTs.

Qualifications

Minimum Qualifications
  • Two years direct service experience which includes providing linkage services, advocacy for low-income populations and working with special needs populations.
  • Experience working with culturally diverse groups.
  • Working knowledge of local community agencies and resources.
  • Understanding of harm reduction and housing first service models.
  • Demonstrable experience with Trauma Informed Care.
  • Experience working with or knowledge of homelessness, mental illness and/or chemical dependency.
  • Ability to clearly set and maintain personal and work boundaries.
  • Demonstrable problem-solving/issue assessment skills, intervention planning/ implementation skills, and crisis intervention/conflict mediation skills.
  • Ability to contribute to a positive work environment that fosters respect, teamwork and excellence.
  • Excellent oral and written communication skills.
  • Basic computer skills with ability to maintain up to date and meticulous records, including case notes and electronic log entries.
  • Ability to work independently and as part of a team.
  • Ability to work within the mission, goals and objectives of Catholic Community Services and Catholic Housing Services.

Preferred Qualifications
  • Experience working in a supportive housing facility or an understanding of residential settings.
  • Bachelor's degree or higher in Social Work or a related field.
  • Five years of experience working in Housing First Programs.
